Pump Flow Setting and Assessment of Unloading in Clinical Practice

In the second lecture Prof. Jacob Møller, Odense University Hospital, Copenhagen University Hospital Rigshospitalet, Denmark, investigates the setting of pump flow and evaluates unloading in clinical practice.

Impella® devices are mainly used in ICU to maintain hemodynamic stability. The usage of left sided transvalvular pumps can lead to left ventricular unloading, by improving the flow and myocardial recovery.

In order to achieve these objectives Prof. Møller advises to use left-sided devices to its fullest capacity possible without suction during treatment. There is little data for pump flow settings during the Impella RP support, but again it is advised to use the device to full support. The onloading is monitored by interpretation of pulse pressure, pulmonary artery wedge pressure and by frequent imaging.

 

Digital Summit: “Use of Impella beyond the cath-lab”

In a recent digital summit, supported by Abiomed, various experts gave lectures on the topic “Use of Impella beyond the cath-lab: open issues and clinical needs”.

The widespread and intense use of the Impella device let experts identify a lack of data regarding the ICU management. On top of that the ICU setting is very heterogenic in different countries and institutions. There is a need to explore this field, for which reason in three different presentations and a panel discussion it is debated how the Impella device and the ICU management of Impella supported patients should be directed.
