Escalation and De-escalation of MCS in Cardiogenic Shock

Dr Letizia Bertoldi, Humanitas Research Hospital, Milan, Italy, talks about escalation and de-escalation in treatment with mechanical circulatory support systems in cardiogenic shock situations. Cardiogenic shock is a clinical entity with many different scenarios and underlying causes. The cardiologist recommends a patient-centered care, a multidisciplinary shock team, continuous multiparametric monitoring and the selection of an appropriate mechanical circulatory support system for this treatment situation. She emphasizes, that the most important aims are to ensure the survival of the patient and to restore cardiac function.

Dr. Bertoldi makes it clear that the choice of the appropriate support system depends on the severity of the cardiogenic shock and the type of cardiac dysfunction. The most commonly used system, she says, is veno-arterial extra-corporal membrane oxygenation (VA-ECMO), although its use is associated with numerous complications. Against this background, Dr Bertoldi evaluates the Impella® heart pump as an alternative. In her presentation, she shows how the Impella® 5.0/5.5 can provide complete cardiac support, as it can reduce the rate of vascular complications. She also highlighted that stability of the device position can be enabled and the heart pump can be used for a longer period of time. Her resume: The technical advantages of the Impella® result in clinical benefits. Infections and organ damage can be prevented. In addition, a complete neurological assessment is possible, and patients can be extubated and mobilized earlier.
